I did a three year residency and felt absolutely prepared for solo practice in a wide variety of departments. There are multiple ways sufficient training is measured during residency, including performing a minimum # of every procedure you are expected perform as an ER doc. Unfortunately, according to @OpenEvidence , people are at a high risk of recurrent dislocation after a first time dislocation. One study showed a recurrence rate of 17% at five years (PMID: 29705013). Another showed recurrent instability, which includes dislocation and subluxation, as high as 63% (PMID: 39348895). If yes for ALL of the following criteria, this has ~96% sensitivity for ruling out PE (if low pretest probability): ✅Age <50 years ✅HR <100 BPM ✅Oxygen sat ≥95% on room air ✅No hemoptysis ✅No hormone use ✅No surgery or trauma requiring hospitalization within the past 4 weeks ✅No prior venous thromboembolism ✅No unilateral leg swelling D-dimer. The sensitivity of a D-dimer test in ruling out pulmonary embolism (PE) in patients with a low pretest probability is 98 to 99%.
